Medicine Procedures and Permission Form
Medicine Procedures:
Please make sure any medication that needs to be given during school hours is brought to the nurse in its original packaging if it’s over the counter. If the medication is a prescription, it must have the pharmacy label on it. This includes inhalers and Epipens. Epipens may only be retained on campus with specific doctor’s instructions. MEDICINE MAY NOT BE SENT WITH A STUDENT...PLEASE HAVE AN ADULT DELIVER THIS DIRECTLY TO THE NURSE. There will also be a form that will need to be filled out for each medication to be given.
